HIV & AIDS Support ServicesBonnie, our nurse, monitors their health indicators and medical treatments. Gayle, our social worker, assesses their basic needs and access to care. They work together with the client to develop a care plan based on his/her specific needs, provide benefit counseling and advocacy, and coordinate with other care providers to assure quality and continuity of care.
AIDS Drug Assistance Program (ADAP)Enrollment in this state program provides needed medications for persons with HIV not covered by insurance. It can also reduce out of pocket expenses by helping with insurance co-pays for many of the medications used to fight HIV and related opportunistic infections. Our case managers are certified enrollment workers for the California State ADAP.
OA HIPPThe State Office of AIDS Health Insurance Premium Payment program can help cover medical insurance premiums for people with HIV who don’t qualify for Medicare or Medi-Cal. These may include individual policies and those purchased through public exchanges like Covered California. If you are no longer working but have continued employer sponsored insurance through COBRA, OA-HIPP may be able to help.
TransportationProvided to clients for medical appointments by staff and volunteers or through use of transit tokens. Gas vouchers are available for clients that are able to transport themselves to medical appointments.
Housing Opportunities for People with AIDS (HOPWA)It can be difficult to focus on meeting your health care needs when you don’t know where you’re going to sleep tonight, or are worried about keeping a roof over your head. HOPWA provides short-term assistance for rent, mortgage or essential utilities intended to prevent or alleviate homelessness of people with HIV or AIDS. Sierra HOPE administers the HOPWA program in Amador, Calaveras, Tuolumne, Alpine, Mono and Inyo Counties.
