Supportive Housing Program
This program provides permanent supportive housing to individuals or families who have become homeless as a result of a physical or mental disability. It consists of six scattered site apartments which are leased to Sierra HOPE.
Supportive services, whether provided by Sierra HOPE or other mainstream community resources, are essential to helping the client access appropriate treatment to manage the symptoms of their illness or disability and live independently. Our case management team works closely with each resident to assess their needs and develop a personalized care plan, and provide on-going monitoring and advocacy to help them access needed services, maintain compliance with medications and treatment plans, and accomplish individual goals towards improved self sufficiency. Sierra HOPE works closely with other service providers including non-profit organizations, local government agencies, medical providers, and mental health services to obtain appropriate referrals and coordinate care. |
